Here's a question -- did punk ever really exist, or was it just some abstract/indefinite thing floated by the likes of Rhodes and McLaren?

Yes. I'd suggest, however, that Malcolm and Bernie were informed by an existing community of kids as much as they tried to reframe that community. A push me/pull you relationship in the heady days of English punk. WIth M & B, a lot of the focus and ambition would have been, if not absent, at least less so. Which would either be a good or a bad thing, depending on your interpretation of what ensued.

UK Punk was a reaction to a specific time and place, 90's UK was a different world to 1976.

Rubbish loads of English bands carried the punk spirit into the 90's what about bands like Mega City Four, Senseless Things and Leatherface they all had punk spirit and were influenced by your own Husker Du, The Replacements, minutemen etc.
